Emergency Contact List & Reporting Chain

Purpose

Provide every Spartan Plumbing employee with the emergency contact numbers and reporting chain for all types of incidents. Save this card to your phone and keep a copy in your truck.

Emergency Numbers

Life-Threatening Emergencies

  • 911 — fire, medical emergency, active danger

Spartan Plumbing Internal

  • Office Main Line: Call dispatch for scheduling, coordination, and general questions
  • Back Office (BOC): (937) 912-1244 — tag Slack first to check if the line is open before calling
  • After-Hours Emergency: Contact the on-call technician or supervisor via Slack

Utilities

  • Gas Company Emergency Line: For gas leaks you cannot resolve or that require utility shutoff
  • Electric Company: For electrical hazards at job sites
  • City of Dayton Water: For water main shutoff requests

Workers' Compensation

  • Ohio BWC: 1-800-644-6292 — Bureau of Workers' Compensation
  • Designated Workers' Comp Clinic: Ask your supervisor for the current clinic location

Key Partners

  • Bates Electric: Electrical referral partner — when customers need electrical work
  • Tradeshift: Purchasing and supply orders

Reporting Chain — Who to Call and When

Safety Incident (injury, fire, gas leak, CO exposure)

  • Call 911 if life-threatening
  • Your direct supervisor
  • Post in Slack #safety-incidents within 1 hour
  • Josh Ferguson if supervisor is unavailable
  • Vehicle Accident

  • Call 911 if injuries or road hazard
  • Your direct supervisor
  • Take photos and exchange insurance information
  • Office will file the insurance claim
  • Customer Property Damage

  • Your direct supervisor immediately
  • Document with photos before cleanup
  • Office handles all insurance communication — do NOT admit fault or make promises
  • Equipment Failure or Safety Concern

  • Your direct supervisor
  • Post in appropriate Slack channel
  • Tag out broken equipment so no one else uses it
  • Important Notes

    • This list should be updated any time contacts change — check with the office quarterly
    • If you cannot reach your supervisor, go up the chain: supervisor then office then Josh
    • When in doubt about whether something is an emergency, treat it as one
    • Always tag Slack before calling the BOC to check if the line is open
