Policy Statement
Spartan Plumbing offers a benefits package to eligible full-time employees. Benefits are administered through Paylocity. This card outlines available benefits and the enrollment process. ## Who This Applies To
All full-time Spartan Plumbing employees. Benefits eligibility begins per the terms stated in the offer letter (confirm current waiting period with Operations Manager). ## Available Benefits
Spartan's current benefits package includes the following. Confirm current plan details and carrier information with Operations Manager or the Operations Manager, as plan specifics change at renewal:
- Health Insurance — Employee health insurance plan with coverage options. Vision and dental options available. - Short-Term and Long-Term Disability — Options available for eligible employees
- Life Insurance — Company-provided or voluntary options (confirm current offering)
- Retirement (Roth IRA) — Company facilitates access; confirm current employer contribution status with Josh
- Paid Time Off (PTO) — See PTO & Time-Off Requests SOP for accrual rates and request process
- Company Vehicle — Field technicians may be assigned a company vehicle depending on role and availability
## Enrollment Process
1. New employees receive benefits enrollment information during onboarding via Paylocity
2. Complete enrollment within the window stated in your offer letter — late enrollment may require waiting until the next open enrollment period
3. Log in to Paylocity (myisolved. com or Paylocity portal — confirm current platform with the Office Admin) to make elections
4. Questions about plan options: contact the Office Admin at the Spartan office
## Changes to Benefits
- Life events (marriage, birth, adoption) allow changes outside open enrollment — notify the Office Admin within 30 days of the event
- Annual open enrollment period: confirm dates with the Josh Ferguson (Owner) each year
## Important Notes
- Benefits are subject to change at each annual renewal — Josh makes final decisions on plan offerings
- Employee contributions are deducted from payroll via Paylocity
- COBRA information is provided upon separation from Spartan
