Workers Compensation Process

Policy Statement

Spartan Plumbing carries workers' compensation insurance as required by Ohio law. Any employee injured on the job is covered by this policy. This card outlines what to do if you are injured at work and how the workers' comp claim process works. ## Who This Applies To

All Spartan Plumbing employees. ## Immediate Steps After a Work Injury

1. Ensure safety first — if the injury is life-threatening, call 911 immediately

2. Notify Operations Manager or Josh — same day, even for minor injuries

3. Seek medical treatment — go to the designated Spartan workers' comp medical provider (confirm current provider with the Operations Manager)

4. Do not use your personal health insurance for a work injury — use workers' comp

## Reporting Requirements

- All work injuries must be reported in writing to Operations Manager within 24 hours — no exceptions

- Fill out Spartan's incident report form (obtained from the Operations Manager)

- Operations Manager files the workers' comp claim with Spartan's insurer (State Auto or current carrier — confirm with the Operations Manager)

## Return to Work

- Obtain a return-to-work release from the treating physician

- Provide release to the Operations Manager before returning to field work

- Light-duty assignments may be available depending on restrictions — Josh makes the final call

- Do not return to unrestricted field work without medical clearance

## Ohio Workers' Compensation

Ohio operates a state workers' comp system (Ohio Bureau of Workers' Compensation). Spartan's premiums are paid to BWC. Claims are filed through BWC or Spartan's TPA (third-party administrator). ## Falsifying a Claim

Falsifying a workers' compensation claim is a criminal offense in Ohio and grounds for immediate termination. ## Important Notes

- Pre-existing conditions that are aggravated on the job may still be covered — report all injuries

- Do not delay reporting — late reports can complicate or deny claims
